Last Month for On-site Biometric Screenings Appointments

Team Plano, this is the last month to schedule on-site biometric screenings! Go to to schedule an appointment as soon as possible. If unable to make one of these reservations, you have the options to request a lab voucher for an off-site screening at LabCorp or have your primary care physician complete a biometric screening form under our Connect4Health Premium Incentive Program.

The health screenings being offered include: total cholesterol, HDL, LDL, triglycerides, glucose, blood pressure, BMI, waist circumference, body composition and (for qualified participants) A1c. You receive results immediately and have the opportunity to discuss them with a trained health care professional when you participate in one of our on-site events.

I’ve already completed my biometric screenings and preventive exam. Now what?

If you are enrolled on the City’s health plan as of January 1, 2018, the only requirements to earn the premium incentive for 2019 are those two steps!

You do not need to notify anyone that you’ve completed the requirements! Connect4Health receives participation reports only. There are no Personal Health Information (PHI)) from United Healthcare or participation reports from US Wellness (no PHI) to confirm who has completed the two required steps.

Senior Wellness Coordinator Michelle Gifford will send notification via email regarding your incentive status in the next few weeks. Watch for her email! As a reminder, if you have a spouse or domestic partner enrolled on the health plan as of January 1, 2018, they also need to complete the two requirements in order to earn the premium incentive savings.
