Done Your Taxes Yet? Don’t Forget Your 1095-C Form.

When it’s time to fill out those wonderful IRS tax returns, it’s important to be sure you have all the right forms. Earlier, Human Resources released information that every 2018 health benefit eligible employee needs before filing this year.

The 1095-C tax forms, required under the Affordable Care Act, are ready and can be accessed remotely. If you consented to receive your 1095-C electronically, your form is now available in PeopleSoft.

  • Select the Benefit Details tile from the Employee Self-Service Menu
  • Choose Affordable Care Act and View 1095-C
  • Click View Form to print a copy for your tax records.

NOTE: if your form does not show immediately, you need to disable the pop-up blocker. A quick way to do this on your computer is to hold the Ctrl key and click View Form at the same time.

If you did not check “yes” to receive the 1095-C form electronically, your form is mailed to the address listed in PeopleSoft.

If you wish to receive the form electronically in PeopleSoft:

  • From the Employee Self-Service Menu select the tile titled “Benefit Details”
  • Choose Affordable Care Act and 1095-C Consent
  • Select to receive the form electronically
