Biometric screening checklist

The City is committed to the health and wellness of every employee and is again offering onsite biometric screenings. As you remember, completing a biometric screening is one of only two 2020 Connect4Health (C4H) Premium Incentive requirements. Now is the time to get ahead of the September 30, 2019 deadline and schedule a screening for March or April.

Go to the Plano Biometric Screening Options webpage to select an onsite biometric screening appointment at one of several convenient locations. If you are not able to attend an onsite screening, you also have the options to:

  • Request a lab voucher for offsite screenings at LabCorp
  • Print a Physician Bio-screening Form for the doctor to complete at your annual exam

The C4H biometric onsite screenings include: total cholesterol, HDL, LDL, triglycerides, glucose, blood pressure, BMI, waist circumference, body composition and A1c for qualified participants. When you participate in one of our onsite events, you’ll receive your results immediately and be able to discuss them with a trained healthcare professional.

NOTE: By completing a biometric screening, you will have satisfied 1 of 2 steps to earn your health insurance premium discount in 2020! This applies to employees and retirees, plus all spouses and domestic partners who are on the medical plan as of January 1, 2019.
