Use Communitas to Make Hospital Stays Less Overwhelming!

There are usually tons of questions and concerns whenever you or a loved-one is in the hospital for any length of time. The City now offers an invaluable resource to help you survive the physical, emotional and financial trauma associated with this kind of medical treatment. Communitas is an onsite case management system to help with every aspect of hospitalizations. It’s a free benefit for employees and dependents, plus pre-65 aged retirees on our health plan.

This program is designed for patients who are hospitalized for a variety of conditions. Communitas OnSIGHT Health RN Case Managers establish relationships with the family and patient to conduct a thorough onsite analysis of all the factors impacting your care and recovery.  

What’s different about Communitas?
Unlike traditional care management services, Communitas certified nurse case managers come to you (or insured family member) to act as your eyes and ears during a hospital stay.

What can Communitas do for you?

  • Provide onsite RN Case Managers who focus on your “before and after” hospital care – a BOOTS on the GROUND approach
  • Help navigate the healthcare system waters (managing costs and billing) and manage your healthcare dollars
  • Lessen the risk of complications with your overall treatment with the best possible recovery in mind

How does Communitas work?

  • Nurses do onsite reviews to create a detailed medical assessment of patient/family, physicians, providers, facilities and business office
  • Explore the family dynamics to determine what happens when patients are discharged
  • Provides info about condition/treatment options and community resources for patient/family
  • Case managers work with hospital business offices on discounts and verifies billing
  • OnSIGHT Health nurses coordinate quick payments based on your case review and more
