Fire Rescue Community Paramedic Program

Did you know Plano Fire-Rescue paramedics take care of patients before anyone calls 9-1-1?

Plano Fire-Rescue’s Community Paramedic Program is part of its new Community Health Systems. Taking a more holistic healthcare approach, the program is a collaboration with local hospitals and healthcare agencies. Along with exceptional emergency medical services Plano Fire-Rescue provides, the Community Paramedic Program offers a higher level of overall care:

  • In-home health maintenance
  • Injury and fall prevention education
  • Wellness visits

The Community Paramedic Program renders a variety of proactive, in-home healthcare services to at-risk residents suffering from chronic illnesses. This help is provided before there’s a need to call 9-1-1. Patients are referred to the program by hospitals, healthcare providers and social workers or are chosen because of their frequent use of the 9-1-1 system.

Plano Fire-Rescue regularly visits patients enrolled in the program to help them comply with post-hospital treatment plans for various diseases, including congestive heart failure, chronic obstructive pulmonary disease, pneumonia, geriatric fractures and asthma.

The social service component of the program identifies other health and quality-of-life needs, like home meal delivery, assistance in getting prescriptions filled and help with transportation to doctor appointments. These unfulfilled needs negatively impact the patient’s medical condition. When needs are identified, Plano Fire-Rescue matches patients with appropriate community resource partners for further assistance.
